HISTORICAL NOTE.
The Evangelical Church in North Andover, Mass., was or- ganized September 3, 1834. It consisted of thirty-one members, -seven males, and twenty-four females. Its first and present House of Worship was dedicated the same day the Church was organized. Rev. WARREN FAY, D. D., of Charlestown, preach- ed the Sermon ; Rev. THos. H. SKINNER, D. D., LL.D., of Andover, Theo. Sem., offered the Dedicatory Prayer; Rev. MIL- TON BADGER, D. D., of Andover, gave the Fellowship of the Churches.
In 1839 the Meeting-house was enlarged sufficiently to make room for twenty-eight additional pews, which, with those origin- ally in it, furnish seats for three hundred and fifty persons.
A Sabbath School was organized Sabbath, Sept. 7, 1834, and has been sustained without interruption from that time to the present.
Though feeble, at first, numerically and pecuniarily, so much so as to need the aid of the Home Missionary Society, in 1841 the Church and Society became self-supporting, and within the last five years have contributed more than twenty-five hundred dol- lars to Benevolent Objects.
The Parish includes about one hundred families, averaging three persons in a family.

FORM OF ADMISSION TO THE CHURCH.
ADDRESS.
BELOVED FRIENDS : You present yourselves before God and these witnesses to make a solemn profession of your Chris- tian Faith, and to enter into Covenant with Christ and His Church in this place. We trust you have duly considered the nature of this profession, and the new relations you will hence- forth sustain, and that you are prepared by Divine grace, to offer yourselves as living sacrifices holy and acceptable to God through Jesus Christ.
Attend then to the
CONFESSION OF FAITH.
You believe in the one only living and true God,-the Father, Son, and Holy Ghost,-who created, and upholds, and governs all things according to the wise and eternal counsel of His own will.
You believe that the Bible is the inspired word of God, the sufficient and only rule of faith and practice.
You believe that God created man originally, holy ; that he fell; and that in consequence of the fall all men are sinners.

CONFESSION OF FAITH.
You believe that Christ took upon himself the form of man, that he died, rose again, and ascended to the right hand of God, where he ever liveth to make intercession for us; that by his obedience, sufferings, and death, he made a complete atonement, which is the only ground for the forgiveness of sin; that pardon and eternal life are sincerely offered to all upon condition of repentance and faith in Christ; and that those who repent and believe will be kept by the power of God through faith unto salvation.
You believe that Christ has established a visible Church in the world, the terms of admission to which, are,-credible evi- dence of regeneration by the Holy Ghost,-and a public profes- sion of faith in the Redeemer ; that Baptism and the Lord's Supper are ordinances of the Church to be observed to the end of time; that none but members of the visible Church, in regu- lar standing, have a right to partake of the Lord's Supper; and that they only, and their households, are proper subjects of Baptism.
You believe also in the Resurrection of the dead ; in a day of final judgment, in which all will receive according to their works ; that the wicked will go away into everlasting punish- ment, and the righteous into life eternal.
Experimentally, you believe that God has wrought in you genuine repentance of sin ; that he has renewed your hearts ; that he has justified and accepted you, and sealed you heirs of heaven.
Thus, according to your understanding of the truth, and according to your personal faith in Christ, you (severally) profess to believe.

COVENANT.
You will now enter into solemn covenant with God and this Church.
COVENANT.
In the presence of God, and these witnesses, you do now avouch the Lord Jehovah, Father, Son, and Holy Ghost, to be your God, the Supreme Object of your affection, and your portion forever. You cordially acknowledge the Lord Jesus Christ to be your Redeemer, and the Holy Spirit to be your Sanc- tifier, Comforter and Guide. You cheerfully devote yourselves to God in the Covenant of his grace; consecrating all your powers, and faculties, and possessions, to his service and glory. And you promise, through the assistance of his Spirit, that you will cleave to Him as your Chief Good; that you will give diligent attention to his word and ordinances, in secret, in the family, and in the house of God; that you will seek the interest and the honor of his kingdom; and that henceforth denying all ungodliness, and every worldly lust, you will live soberly, and righteously, and godly in the world.
You do also cordially join yourselves to this Church, engag- ing' to submit to its rules of government and discipline, to strive earnestly for its peace, edification and purity, and to walk with its members in charity, fidelity and Christian watchfulness.
Into this Covenant you (severally) enter.
We then, the members of this Church, affectionately receive you to our communion, and in the name of Christ, declare you entitled to all the privileges of membership in the Church of God. We welcome you to this fellowship with us in the bless- ings of thegospel; and on our part, engage to watch over you in Christian affection, and to seek your edification so long as you continue to be with us.
And now, beloved in the Lord, let it never be forgotten that

SELF-EXAMINATION.
the vows of God are upon you, and that the obligations which you have ratified and sealed this day, are as lasting as eternity. Wherever you go, they will be with you. They will follow you to the bar of God. They will abide with you, in whatever world you may be, forever. Let it be your constant aim, and earnest prayer to God, that he will help you to fulfill them. May the Lord guide and preserve you till death, and at last receive you and us to that blessed world where our love and joy shall be perfect forever, through the grace of our Lord Jesus Christ. Amen.
SELF-EXAMINATION.
1. Do I habitually attend Public Worship, and invite others to do the same ?
2. Do I habitually attend the Prayer Meeting ?
3. Do I habitually attend the Preparatory Lecture and the Communion ?

12. Am I concerned for the universal spread of the Gospel ; and how do I practically show my concern ?
13. Am I active in devising and executing schemes of good- ness ?
14. Do I honor and love my Pastor for his work's sake, and remember him in my prayers ? Do I tenderly guard his reput- ation ; cheerfully co-operate with him in plans for doing good; and make known to him anything of importance relating to the welfare of the Church and Congregation ?
15. Do I cultivate a fervent affection for the whole family of believers, especially those with whom I am more immediately connected in Church Covenant ?
16. Do I daily practise self-denial for Christ's sake ?
17. Am I careful of my deportment, especially in the pres- ence of those who do not love God; that I do nothing to disgust them with religion; that I say nothing needlessly to wound their feelings, or selfishly to gratify my own; that I avoid all artificial appearances, affectation, bigotry, bitterness, useless dis- putation, formality, distance, moroseness, levity, extravagance, while I maintain an habitual sense of the Divine Presence in my soul, simple trust in God. firmness in the truth, a desire to communicate a knowledge of it, and a readiness to co-operate with others in its advancement ?
