Notes on Deaths and the Causes of Death.
Death-Rates .- In the year 1892, 12,147 deaths were registered in the State, which is equivalent to a death-rate of 18.37 for every 1,000 of population as returned by the census of 1890. Based on the estimated population for 1892, the death rate would be 18.30.
By referring to Table No. 2, it will be seen that the death-rate of Aroostook county was lower than that of any other county, being only 12.48, while the older counties, Cumberland, Kennebec, and York show death-rates of 20.28, 22 32, and 22.20 respectively.
The death-rate in the cities was 20.46 as compared with that of the rest of the State with the cities substracted, which death-rate was 17.46.
Death-Rates Compared .- For the year 1892 the death-rate of Maine was 18.37; of New Hampshire, 20 88 ; of Massachusetts, 20.57 ; of Connecticut, 19. The average death-rate in some Euro- pean countries for the five years, 1887-91, was as follows : England and Wales, 19.0; Ireland, 18.0; France, 21.9 ; German Empire, 23.9; Holland, 20.3; Denmark, 19.1; Norway, 17.1; and for the years 1887-90, Sweden, 16.3 ; Hungary, 32.
In comparing the death-rate of one place with that of another one should be cautious in drawing conclusions. Besides salubrity or insalubrity of climate, other circumstances influence death-rates, particularly age, sex, social condition, and occupation.
Season and Mortality .- The largest number of deaths occurred in January, 1,577, and February and March followed with a dimin- ishing mortality, 1,292 and 1,095 deaths respectively.
The smallest number of deaths in any one month was 790 in November, and December followed with an increase of only 3 over the preceding month.
In the first two months of the year, there were 2,867 deaths to compare with 1,583 in the last two months.
In the first quarter of the year, 3,964 deaths occurred, and in the last quarter, 2,427.

It is thus shown that, in the second quarter, there was a diminu- tion in the number of deaths by more than one thousand and in the two succeeding quarters a smaller diminution in the mortality.
This excessive fatality in the early part of the year represents apparently, not a normal or customary late-winter and early-spring death-rate in the State, but was due mostly, and perhaps wholly, to the wide-spread prevalence of epidemic influenza, and its accom- paniments and sequels.
In our State the lack of the registration of deaths for all pre- ceding time, deprives us of the means of comparing the mortality of this year, 1892, with that of other years; but turning to the registration reports of our sister New England States we are told in that of Massachusetts for the year 1892 :
The greatest number of deaths occurred in January and in the first quarter, and the least number in June and the last quarter. It also appears that, of the whole number of deaths reported as from the acute diseases of the respiratory organs, bronchitis, pneu- monia and pleuritis. more than 27 per cent. occurred in January ; and of the deaths as having been absolutely caused by influenza, more than 59 per cent were reported as occurring in January.
During the twenty-two years, 1871-1892, the quarterly mortality has been largest in the first quarter of the year only thrice,-1879, 1890, and 1892.
The report for Connecticut for the same year says that, "the. greatest mortality was registered in January and reached 1,986. This was caused by the return of the epidemic la grippe, and a very large mortality from acute diseases of the lungs "
It appears not far from the truth to affirm that the epidemic of influenza (la grippe) cost this State 1,000 lives in 1892, and raised the recorded death-rate corespondingly.
Sex and Mortality .- Of the 12,147 decedents, 5,939 were males and 6,175 were females. The sex of 33 was unstated.
Among children dying before they were five years old, 1,320 were males and 1,058 females; on the contrary, at all the age periods from 10 to 100 years and over, there was a preponderance of female deaths, with the exception of the period 70-80 years. See Table No. 26.
Age and Mortality. -- The following shows the number of deaths which occurred within each age period, and the percentage which the deaths in each period bears to the whole number of deaths :

The reader of these numbers and percentages will note the great mortality at the two extremes of life, but this mere fact is not sur- prising. He will, however, see that the proportionate number of deaths of aged people is large. To il ustrate this the following arrangement will serve to compare Maine with Massachusetts as. regards the percentage of deaths at seve al age periods :

This comparison brings out sharply the relatively large number of deaths of aged persons in Maine.
Does this indicate a superior degree of healthfulness in the State? An explanation of only a part of the difference is to be sought in that direction Is it attributable to the influence of the epidemic of influenza and its accompanying diseases of the respiratory organs which proved so fatal to the aged part of the population? The death-rate from influenza was consid- erably greater in Maine than in Massachusetts, but the combined death-rate from influenza, pneumonia, and bronchitis in Maine exceeded that in Massachusetts only as 37 to 36.
It is likely that the proportionately large number of deaths among the aged in our State is due in some measure to the fact that, for years, the emigration of young men and young women from Maine to the Western States and to the larger cities of New England has been large. A proportionately larger number

of elderly persons are therefore left to swell the relative death-rate of the aged and consequently the general death-rate.
Infantile Death-Rates .- In the comparison with Massachusetts it will be noted that the percentage of the deaths of children, when compared with the total mortality, is relatively small. Now, since infants are easily affected by noxious influences, particularly, insa- lubrity of climate and insanitary conditions of domiciles, the infantile death-rate of a country, a State, or a city is often taken as an index of the healthfulness of the place. With due precau- tions against error it may be so used.
The number of infants under one year of age dying in each 1,000 of total population was 2 5 in Maine and 4.7 in Massachu- setts. The death rate of children under five years of age was 3.5 in Maine and 6.8 in Massachusetts.
But a much less crude, and a more trustworthy method of getting the death-rate for any given age period is, not to base it upon the general population, but upon the number of persons living at the particular age-period. As the population by ages at the last cen- sus is not available, it is here assumed that the age distribution of the population of our State differed but little in 1890 from that in 1880. In the following table the death-rates of Maine and of Massachusetts in 1892 are based upon the estimated number in each state of persons living at the four different age-periods :

Thus the table indicates that, among each 1,000 infants living in the State of Maine in 1892, a little over 130 died, and in Massa- chusetts, out of 1,000 infants under one year of age, 216 died. In like manner the death-rates of children from the time of birth until the fifth year, of persons from twenty to fifty years of age, and of those sixty years and upward, are compared.
The Causes of Death .- Of the 12,509 deaths, including still- births, and premature births, the cause of 11,849 was certified, leav- ing 660 of which the cause of death was unspecified or so ill-defined as to preclude satisfactory classification.

By referring to Table No. 23, it will be seen that the causes of death are arranged under eight "Classes," and that the causes of death in some of the classes are distributed under several "Groups." The following notes on some of the classes and groups of diseases and individual causes of death will be arranged in accordance with the classification shown in this Table 23 and the classification of the causes of death shown in the latter part of this report.
Zymotic Diseases .- The number of diseases under this class in Table 23 is largely increased by including tuberculosis in its various forms, and syphilis under the group : Specific Neoplastic Diseases.
The death-rate from the zymotic diseases, as well as the infantile death-rate, is much used as an index of the healthfulness of states and municipalities, and of the efficiency of their sanitary organiza- tions. Applying it as a sanitary yardstick to our State, its various counties, and its cities, is an interesting diversion, and extended over a series of years will give results of value. The following shows the death-rate from the principal zymotic diseases. to wit : Small-pox, measles, scarlet fever, typhoid fever, whooping cough, diphtheria and croup, and the diarrhoeal diseases :
