Parents and others are requested to carefully examine the above lists and if any errors or omissions are discovered report them at once to the town clerk, that the record may be cor- rected, as it is important that the Records of Vital Statistics should be complete. Herewith are extracts from the Revised Laws of Massachusetts.
CHAPTER 280. ACTS OF 1912.
SECTION I .- Physicians and midwives shall, within forty- eight hours after the birth of every child in cases of which they were in charge, mail or deliver to the clerk or registrar of the town or city in which the birth occurred a notice stating the date and place of birth, giving the street number, if any, the number of a ward in a city and the family name. Failure to mail or deliver the said notice shall be punished by a fine not exceeding twenty-five dollars for each offense. The notice required by this section need not be given if the notice required by the fol- lowing section is given within forty-eight hours after the birth occurs.
SECTION 2. Physicians and midwives shall make and keep a record of the birth of every child in cases of which they were in charge and shall, within fifteen days after the birth, mail or deliver to the clerk or registrar of the city or town in which the birth occurred a report of the birth, stating the date and place, the name, if any of the child, its sex and color, and the names, ages, place of birth, occupations and residence of the parents, giving the street number, if there be any, and the number of a ward in a city, the maiden name of the mother, and whether or or not the physician or midwife signing the. birth return person- ally attended the birth. If the child is illegitimate, the name and other facts relating to the father shall not be stated except at the request in writing of both the father and mother filed with the return. The record to be kept by the physician or midwife, as above provided, shall also contain the facts hereby required to be reported to the city or town clerk. The fee of the physician or midwife shall be twenty-five cents for every

birth so reported, which shall be paid by the city or town where the report is made, upon presentation to the city or town treas- urer of a certificate from the city or town clerk stating that the said birth has been properly reported to him. The report required to be made by this section, is in addition to the report required to be made by the preceding section, and as above provided, if made within forty-eight hours of the birth, the report required by the preceding section shall not be required.
A physician or midwife who neglects to make and keep the record hereby required, or who neglects to report in the manner specified above, each birth within fifteen days thereafter, shall for each offense forfeit a sum not exceeding twenty-five dollars. The city or town clerk or registrar shall file daily with the board of health a list of all births reported to him, giving the follow- ing facts : date of birth, sex, color, family name, residence, ward, physician or midwife.
SECTION 6. Chapter 29, Revised Laws. Parents within forty days after the birth of a child, and every householder with- in forty days after a birth in-his house shall cause, notice thereof to be given to the clerk of the city or town in which such child was born.
Blanks for the return of births can be obtained at the office of the town clerk.

TOWN WARRANT.
COMMONWEALTH OF MASSACHUSETTS.
Worcester ss.
To either constable of the town of Milford in said county,
Greeting:
In the name of the Commonwealth aforesaid, you are hereby required to notify and warn the inhabitants of said town, qualified by law to vote in elections and in town affairs, to meet at the Town Hall in said town, on Monday, the third day of March, A. D., 1913, punctually at 5.45 o'clock in the forenoon to act upon the following articles, namely :-
[The polls may be closed at four o'clock in the afternoon on said day of meeting.]
ARTICLE 1. To choose a moderator to preside at said meeting.
ARTICLE 2. To choose all necessary town officers, a town clerk, town treasurer, tax collector, auditor, three selectmen, (for one year), one assessor, (three years), three overseers of the poor, (one year), one sewer commissioner, (three years), two members of the school committee, (three years), one mem- ber of the board of health, (three years), two trustees of pub- lic library, (three years), two trustees of Vernon Grove ceme- tery, (three years), one park commissioner, (three years), and five constables, (one year), to be voted for on ballots prepared and furnished in accordance with the provisions of Chapter 560 Acts of 1907, and acts in amendment thereof and addition thereto. Also upon said ballots the voters shall give in their votes, "Yes" or "No" in answer to the question : "Shall licen- ses be granted for the sale of intoxicating liquors in this town?"
ARTICLE 3. To hear and act upon the reports of the selectmen, overseers of the poor, school committee, treasurer,

tax collector, auditor, town clerk, sewer commissioners, trus- tees of public library, trustees of Vernon Grove cemetery, park commissioners, town solicitor, engineers of the fire de- partment, assessors, chief of police, board of health, inspector of animals, sealer of weights and measures, and any other offi- cers or conimittees of said town.
ARTICLE 4. To raise and appropriate such sum or sums of money as may- be necessary to defray town expenses for the year ensuing.
ARTICLE 5. To see if the town will vote to authorize the treasurer to borrow money in anticipation of the taxes of the current year.
ARTICLE 6. To determine when all taxes shall be col- lected and paid into the treasury the year ensuing, and fix the compensation of the collector of taxes for collecting the same.
ARTICLE 7. To see if the town will vote to raise and appropriate the sum of one hundred and sixty-five dollars ($165.00) towards defraying the expenses of Memorial Day.
ARTICLE 8. To see if the town will vote to authorize the selectmen to take charge of all legal proceedings for or against the town.
ARTICLE 9. To see what action the town will take in re- gard to lighting its streets, and raise and appropriate money for the same.
ARTICLE 10. To see if the town will vote to contract with the Milford Water Company for its water supply, or take any action in relation thereto.
ARTICLE 11. To see if the town will vote to raise and appropriate the sum of one thousand dollars ($1,000.00) for the purpose of building a fire station at North Purchase, so called, or take any action in relation thereto.
ARTICLE 12. To see if the town will vote to anthorize the assessors to publish a list of real and personal estates, and the valuation thereon, raise and appropriate money therefor, or take any action in relation thereto.
ARTICLE 13. To see if the town will vote to instruct its

selectmen to appoint an inspector of wires, determine his salary, or take any action in relation thereto.
ARTICLE 14. To see if the town will vote to raise and appropriate the sum of seventy-five thousand dollars ($75,000 .- 00), or will authorize the town treasurer to issue bonds or notes to an amount not exceeding seventy five thousand dola lars ($75,000.00), at a rate of interest not exceeding four and one-half per cent per annum, pay able semi-annually, for the purpose of buying land and the erection thereon of a school building, or take any action in relation thereto.
ARTICLE 15. To see if the town will vote to raise and appropriate the sum of four thousand dollars ($4,000.00) for the purpose of rebuilding West street, from a point at or near Highland street, to Congress street, or take any action in re- lation thereto.
ARTICLE 16. To see if the town will vote to raise and appropriate the sum of twenty-five hundred dollars ($2,500.00) for maintaining its sewer disposal plant, or take any action in relation thereto.
ARTICLE 17. To see if the town will vote to raise and appropriate the sum of four thousand dollars ($4,000.00) for the erection of a public comfort station, or take any action in relation thereto.
ARTICLE 18. To see if the town will vote to raise and appropriate the sum of fifty dollars ($50.00) for the care of North Purchase cemetery, or take any action in relation thereto.
ARTICLE 19. To see if the town will vote to raise and appropriate the sum of three hundred dollars ($300.00) for public band concerts, or take any action in relation thereto.
ARTICLE 20. To see if the town will vote to build a curbed sidewalk on the southerly side of Poplar street, from Claflin street to Myrtle street, raise and appropriate the sun of five hundred dollars ($500.00) for the same, or take any ac- tion in relation thereto.
ARTICLE 21. To see if the town will authorize the select- men and appoint them its agents for the purpose of executing

proper deeds and any other releases necessary for the carry- ing out of a compromise settlement in case of Nason vs. Town of Milford, or take any action in relation thereto.
ARTICLE 22. To see if the town will vote to instruct the selectmen to locate an electric light on Green street, and raise and appropriate money for the same, or take any action in re- lation thereto.
ARTICLE 23. To see if the town will vote to build a curbed sidewalk on Meade street, raise and appropriate money therefor, or take any action in relation thereto.
ARTICLE 24. To see if the town will vote to buy the Milford Electric Light & Power Co, raise and appropriate money for the purpose, or take any action in relation thereto.
ARTICLE 25. To see if the town will vote to purchase the Milford Water Company, or appoint a co.nmittee to inves tigate the matter and report at the next town meeting, raise and appropriate money for the purpose or take any action in relation thereto.
And you are hereby directed to serve this warrant by posting up attested copies thereof at each of the public meet- ing houses and at the post office in said town, also cause at- tested copies to be published in the Milford Daily News, Mil- ford Daily Journal and Milford Gazette, newspapers printed in said town, two Sabbaths at least before the time set for said meeting.
Ilereof fail not, and make due return of this warrant with your doings thereon to the clerk of said town at the time of meeting aforesaid.
