To provide for biennial elections of state officers, council- lors and members of the General Court.

term for which he was elected be appointed to any office crea- ted or the emoluments whereof are. increased during such terin, nor receive additional salary or compensation for service upon any recess committee or commission except a committee appointed to examine a general revision of the statutes of the Commonwealth when submitted to the General Court for adoption.


ARTICLE OF AMENDMENT No. 19.


On or before January first, nineteen hundred twenty one, the executive and administrative work of the Commonwealth shall be organized in not more than twenty departments, in one of which every executive and administrative office, board and . commission, except those officers serving directly under the Governor or the council, shall be placed Such depart- ments shall be under such supervision and regulation as the General Court may from time to time prescribe by law

Milford, November 5, 1918.


In pursuance to the foregoing warrant the inhabitants of the town of Milford, qualified by law to vote in elections and in town affairs, met in Dewey Hall in said town on Tuesday, the fifth day of November, A. D., 1918, punctually at 5.45 o'clock in the forenoon, and were called to order by the town clerk who read the warrant and the officer's return thereon.


In accordance with the provisions of Chapter 835, Acts of 1913, and acts in amendment thereto, sealed packages said to contain 3200 official ballots, specimen ballots, cards of in- structions, penalty cards, and copies of the proposed amend- ments of the Constitution were delivered to Thomas J. Nu- gent, presiding election officer.


The seal was then publicly broken, the package opened.

and the ballots delivered to the ballot clerks and their receipt given to the town clerk.


One card of instructions, one penalty card and one copy of the proposed amendments of the Constitution were posted ' in each marking compartment, and five specimen ballots, five cards of instructions, five penalty card-, and five copies of the proposed amendments of the Const tution were posted in the hall outside the rail.


The ballot box required by law to be used in elections was then publicly opened and found to be empty, the regis- ter of the box was set at 0: 00, the box locked and the keys delivered to Jeremiah T. Murphy, constable in attendance at said meeting.


Harold K. Bullard and Dominick J Lang, tellers appoint- ed by the selectmen to check the voting list at the polls, and? Ernest A. Whitney and James F. Egan, ballot clerks, and Al- fred B. Cenedella and James F. Baxter, deputy ballot clerks, were swoin to a faithful performance of their duties by the town clerk.


The polls were opened at 5.52 o'clock A. M.


Voted : To close the polls at eight o'clock P. M.


Due notice having been given the poll at eight o'clock F. M. were declared closed.

During the voting the ballot box was opened, by the unanimous consent of the selectmen and the town clerk,

eleven times, for the purpose of taking out the ballots and sorting and counting the same, first at 11 30 A. M when the box registered 450, at 1 50 p M. when it registered 700, at 2.40 P. M when it registered 800, at 4.15 p. M. when it regis - tered 950 at 4.37 p M. when it registered 1004, at 5 p. M. when it registered 1020, at 5.50 P. M. when it registered 1200, at 6 53 P M when it registered 1401, at 7.35 P. M. when it registered 1701, and at the close of the polls, 8 P M. when it registered 1818.


The count on the ballot box did not agree with the num- ber of names checked at the polls and at the tables of the bal- lot clerks.


The names of those on the lists of voters which were checked and audibly counted were found to be as follows : List at the polls 1817. List at the tables 1817.


The following additional tellers appointed by the select- men were sworn to a faithful performance of their duties by the town clerk :

ARTICLE OF AMENDMENT.


The conservation, development and utilization of the agri- cultural, mineral, forest, water and other natural resources of the Commonwealth are public uses, and the general court shall have the power to provide for the taking upon payment of just compensation therefor, of lands and easements or inter- ests therein, including water and mineral rights, for the pur- pose of securing and promoting the proper conservation, development, utilization and control thereof, and to enact leg- islation necessary or expedient therefor.

The ballots. were then de- livered into the custody of the town clerk, and also the voting lists, in conformity with law in such cases made and provi- ded.


The unused ballots remaining, and all soiled and can- celled ballots certified by the ballot clerks to be such were sealed in a package and delivered to the town clerk as the law provides.

VITAL STATISTICS.


Parents and others are requested to carefully examine the following lists and if any errors or omissions are discovered. report them at once to the town clerk, that the record may be corrected, as it is of importance that the record of Vital Sta- tistics should be complete. Herewith are extracts from the Revised Laws of Massachusetts, Chap. 280, Acts of 1912.


SECTION 1. Physicians and midwives shall, within forty- eight hours after the birth of every child in cases of which they were in charge, mail or deliver to the clerk or registrar of the city or town in which the birth occurred, a notice stat- ing the date and place of the birth, giving the street number, if any, the number of a ward in a city and the family name. Failure to mail or deliver the said notice shall be punished by a fine not exceeding twenty five dollars for each offense. The notice required by this section need not be given if the notice required by the following section is given within forty-eight hours after the birth occurs.


SECTION 2. Physicians and midwives shall make and keep a record of the birth of every child in cases of which they were in charge and shall, within fifteen days after the birth, mail or deliver to the clerk or registrar of the city or town in which the birth occurred a report of the birth, stating the date and place, the name, if any, of the child, its sex and color, and the names, ages, places of birth, occupations and residence of the parents, giving the street number, if there be any, and the number of the ward in a city, the maiden name of the mother, and whether or not the physician ( r midwife signing the birth return personally attended the birth. If the child is illegitimate, the name and other facts relating to the father shall not be stated except at the request in writing of both the father and mother filed with the return. The record to be kept by the physician or midwife, as above provided,

shall also contain the facts hereby required to be reported to the city or town clerk. The fee of the physician or midwite shall be twenty-five cents for every birth so reported, which shall be paid by the city or town where the report is made, upon presentation to the city or town treasurer of a certi- ficate from the city or town clerk stating that the said birth has been properly reported to him. The report re- quired to be made by this section is in addition to the report required to be made by the preceding section, and as above provided, if made within forty-eight hours of the birth, the report required by the preceding section shall not be re- quired. A physician or midwife who neglects to make and keep the record hereby required, or who neglects to report in the manner specified above each birth within fifteen days thereafter, shall for each offense forfeit a sum not exceeding twenty- five dollars. The city or town clerk or registrar .shall file daily with the local board of health a list of all births re- ported to him, giving the following facts : date of birth, sex, color, family name, residence, ward, physician or midwife.
