TOWN WARRANT COMMONWEALTH OF MASSACHUSETTS


Worcester, ss.


To either Constable of the Town of Milford in said County, Greeting :


In the name of the Commonwealth aforesaid, you are hereby required to notify and warn the inhabitants of the Town of Milford qualified by law to vote in Town Affairs, to meet in the Town Hall in said Town on Friday, the fifteenth day of October, A. D. 1937, at eight o'clock in the evening, then and there to act upon the following articles, namely :-


Article 1. To hear and act upon the reports of any Com- mittees of said Town:


Article 2. To see if the Town will vote to amend the vote passed under Article 2 of the Warrant for the Special Town Meeting held on September 8, 1937, by striking out the words "Ninety thousand dollars ($90,000.00)" and substi- tuting therefor the words "One Hundred and One Thousand Five Hundred Dollars ($101,500.00)", and by striking out the words "Twenty Five Thousand Eight Hundred Thirty

Nine and 64/100ths Dollars ($25,839.64)" and substituting therefor the words "Thirty seven Thousand Three Hundred Thirty-nine and 64/100 Dollars ($37,339.64)" so that said vote shall read :


That the Town appropriate the sum of One Hundred and One Thousand Five Hundred Dollars ($101,500.00) includ- ing the amount of Sixty-four Thousand One hundred and Sixty and 36/100 Dollars ($64,160.36) accruing to the Town on account of the adjustment of the fire loss at the High School and Thirty-seven Thousand Three Hundred and Thirty-nine and 64/100 Dollars ($37,339.64) from unappro- priated funds in the Treasury to meet the costs of the re- construction and furnishing of said school and of Architect's fees and all other incidental expenses connected with the fire loss thereon, the same to be expended under the super- vision of a Building Committee of fifteen (15), consisting of the School Committee, the Board of Selectmen, the Town Counsel, and five (5) Town Meeting Members, one from each Precinct.


And you are hereby directed to serve this Warrant by publishing at least seven days before said meeting an at- tested copy of this Warrant in the Milford Daily News, a newspaper published and having a general circulation in the Town of Milford, and by posting at least seven days before said meeting attested copies of this warrant in five or more public places located in said Milford.


Hereof Fail Not, and make due return of this Warrant with your doings thereon to the Clerk of said Town at the time of meeting aforesaid.

REPORT OF THE FINANCE COMMITTEE


The Committee recommends the passing of the following vote :


That the vote passed under Article 2 of the warrant for the special town meeting held on September 8, 1937, be amended by striking out the words "ninety-thousand dol- lars ($90,000.00)" and by substituting therefor the words "ninety-four thousand dollars ($94,000.00)", and by strik- ing out the words, "twenty-five thousand eight hundred thirty-nine and 64/100 dollars ($25,839.64) and substituting therefor the words "twenty-nine thousand, eight hundred thirty-nine and 64/100 dollars ($29,839.64) and by striking out the words "and furnishings" and by striking out the words "the fire loss thereon", and inserting after the word "with" the words "said reconstruction", so that the vote shall read :-

Vital Statistics.


Parents and others are requested to carefully examine the following lists and if any errors or omissions are dis- covered report them at once to the Town Clerk, that the record may be corrected, as it is of importance that the record of Vital Statistics should be complete. Herewith are extracts from the General Laws of Massachusetts, Chapter 46.


Section 3. Every physician or hospital medical officer registered under Section nine of Chapter one hundred and twelve, in this chapter called officer, shall keep a record 'of the birth of every child in cases of which he was in charge, showing date and place of birth, the name, if any, of the child, its sex and color, the name, age, birthplace, occupation and residence (including the street, number, if any, and the ward number, if in a city) of each parent, the maiden name of the mother and the name of the physician or officer, if any, personally attending the birth. If the child is illegitimate, the name of and other facts relating to the father shall not be set forth except upon written request of both the father and mother. Said physician or officer shall, within fifteen days after such birth, mail or deliver to the clerk or registrar of the town where such birth occurred, a report stating the facts herein above required to be shown on said record and also the said written request, if any; provided, that if said report is not so made within forty- eight hours after such birth, said physician or officer shall, within said forty-eight hours, mail or deliver to said clerk or registrar a notice stating the date and place of birth, the street number, if any, the ward number, if in a city, and the family name. Upon presentation to him of a certificate of the Town Clerk stating that any such birth has been duly reported, the Town Treasurer shall pay to such physician or officer a fee of twenty-five cents for each birth so re- ported. Any physician or any such officer violating any provision of this section shall forfeit not more than twenty- five dollars.


Section 4. A member or servant of a family in which a child is born, having knowledge of the facts required for

record relative to such births, shall furnish the same upon request of the clerk of the town where such child was born or its parents reside, or of any person authorized by him. Such member or servant refusing to furnish such facts shall be punished by fine of not exceeding ten dollars.


Section 6. Parents, within forty days after the birth of a child, and every householder, within forty days after a birth in his house, shall cause notice thereof to be given to the clerk of the town where such child is born. Every house- holder in whose house a death occurs and the oldest next- of-kin of a deceased person in the town where the death occurs shall, within five days thereafter, cause notice there- of to be given. to the board of health, or, if the selectmen constitute such board, to the town clerk. The keeper, super- intendent or person in charge of a workhouse, house of correction, prison, reformatory, hospital, almshouse or other institution, public or private, which receives inmates from within or without the limits of the town where it is located shall, when a person is received, obtain a record of all the facts which would be required for record in the event of the death of such person, and shall, on or before the fifth day of each month, give notice to the town clerk of every birth and death among the persons under his charge during the preceding month.


Blanks for returns of births and deaths may be obtained at the office of the town clerk.
