Parents and others are requested to carefully examine- the above lists and if any errors or omissions are discovered report them at once to the town clerk, that the record may be- corrected, as it is of importance that the records of Vital. Statistics should be complete. Herewith are extracts from. the Revised Laws of Massachusetts, Vol. 1, Chapter 29.
Section 3. Physicians and midwives shall on or before the fifth day of each month report to the clerk of each city or town a correct list of all children born therein during the pre- ceding month at whose birth they were present, stating the date and place of each birth, the name, if any, of the child, its- sex and color, and the name, place of birth, and residence of the parents, the maiden name of the mother, and the occupa-
tion of the father. If the child is illegitimate, the name of, and other facts relating to, the father shall not be stated, ex- cept at the request in writing of both father and mother filed with the return. The fee of the physician or midwife shall be twenty-five cents for each birth so reported, which shall be paid by the city or town where the report is made, upon pre- sentation to the city or town treasurer of a certificate from. the city or town clerk stating that said births have been properly reported to him. A physician or midwife who. neglects to report such lists on or before the fifteenth day of the month shall for each offence forfeit not more than twenty .- five dollars.
Section 6. Parents within forty days after the birth of a child, and every householder, within forty days after a birth in his house, shall cause notice thereof to be given to the clerk of the city or town in which such child is born.

Greeting :
In the name of the Commonwealth aforesaid, you are hereby required to notify and warn the inhabitants of said town, qualified by law to vote in elections and in town affairs, to meet at the Town Hall in said town, on Monday, the first day of March, A. D., 1909, punctually at six o'clock in the forenocn, to act upon the following articles, namely :-
[The polls may be closed at four o'clock in the afternoon on said day of meeting.]
ARTICLE 1. To chocse a moderator to preside at said meeting.
ARTICLE 2. To choose all necessary town officers for the year ensuing : A town clerk, town treasurer, tax collector, auditor, three selectmen, one assessor (three years), one assess- or (two years), to fill vacancy, three overseers of the poor, one sewer con. missioner (three years), two members of the school committee (three years), one member of the board of health (three years), two trustee of public library (three years), one trustees of public library (two years), to fill va- cancy, two trustees of Vernon Grove cemetery (three years), and five constables, to be voted for on ballots prepared and furnished in accordance with the provisions of Chapter 560, Acts of 1907, and Acts in amendment thereto, also upon said ballots the voters shall give in their votes "Yes" or "No" in answer to the following questions, "Shall Chapter 513 of the Acts of the year nineteen hundred and eight, requiring certain cities and towns to provide public playgrounds be ac-

cepted by this town?" "Shall Section 20, Chapter 106 of the Revised Laws, an act 'providing thit eight hours shall constitute a day's work for all liborers, workmen und mechin- ics who are employed by this town' be accepted by this town?" "Shall licenses be granted for the sale of intoxicating liquors in this town?" Also upon sepirate billots the voters shall give in their votes, "Yes" or "N )" in answer to the foll ) ving question. "Shall an act passed by the General Court in the year nineteen hundred and eight, entitled, 'An act to provide for the protection of forest or sprout lands from fire' be ac- cepted by this town?"
ARTICLE 3. To hear and act upon the reports of the select men, school committee, overseers of the poor, treasurer, engineers of the fire department, tax collector, trustees of public library, trustees of Vernon Grove cemetery, and any other officers and committees of stid towa.
