REPORT OF THE BOARD OF HEALTH NURSE

Dear Mr. Frost:
I herewith respectfully submit my twenty-fourth annual report as Board of Health Nurse for the town of Douglas.
Each year seems to present a special challenge to me, for each year through practical experience and through special preparation for my job through the medium of meetings, courses, association with people dedicated to public health, books-all of these help to motivate me to share my knowledge and to endeavor to meet the needs of the community, so that Douglas will contribute its full share in trying to meet the goals established by the World Health Committee.
I present the following statistics for your approval and the approval of the citizens of Douglas.
COMMUNICABLE DISEASE PREVENTION
All clinics are held under the direction of Dr. R. H. Spooner and the Board of Health Nurse.
Oral Polio Vaccination:
The State Department of Public Health made available Types I, II, and III of Sabin oral polio vaccine for children from three months through 18 years of age. The local Board of Health pur- chased Types I, II, and III oral polio vaccine for adults and charged 25 cents per dose, which paid for the vaccine. Town employees and volunteer workers were offered the vaccine without charge as a cour- tesy from the local health board. The physician was paid from local Board of Health funds.
TYPE I. On January 22 and 23, Type I polio vaccine was given at the high school, Douglas Center, and East Douglas Elementary buildings. On January 30 and February 4 clinics were held at the Town Hall from three to five o'clock and from seven to eight o'clock for pre-school children and adults.
TYPE II. On March 19 and 20, Type II polio vaccine clinics were held at the high school, Douglas Center, and East Douglas

Elementary buildings. On March 21 a clinic for Type II vaccine for pre-school children and adults was held at the Town Hall from ten to 12, from one to five, and from seven to eight o'clock.
TYPE III. On May 21 and 22, Type III polio vaccine was given at the high school, Douglas Center, and East Douglas Elementary buildings. On May 23 and on June 3rd, 5th, 7th, and 8th, clinics for Type III vaccine were held at the Town Hall for pre-school chil- dren and adults. These dates also included make-up clinics for those pople who had not yet received Type I or Type II polio vaccine.
The following is the breakdown as to the number of doses of oral polio vaccine that were received by each age group. Pre-School Children:

To summarize the above figures, a total of 3,365 doses of the three types of oral polio vaccine were received by the people of Doug- las in 1963. A total of 1,254 residents of Douglas in all age groups have now completed the full series of three doses necessary to pro-

vide complete immunization against polio. More polio vaccine clinics will be held during 1964, and it is our hope that all residents who have not already done so will take advantage of this opportunity to obtain complete polio immunization.
DIPHTHERIA, WHOOPING COUGH, AND TETANUS IMMUNIZATIONS
On April 2nd, 10th, and 30th, May 28th, and June 25th, clinics for diphtheria, whooping cough, and tetanus immunizations and for smallpox vaccinations were held at the Town Hall and the High School from 9 to 12 o'clock.

FLU VACCINATIONS
On September 16 and 17, and on November 19 and 20, Dr. Spooner administered flu vaccine to school personnel, town em- ployees, and volunteer workers. The local health department pur- chased the vaccine commercially. A nominal charge of one dollar per person was made in order to help defray the expenses of this clinic, namely the cost of the vaccine and the doctor's fee.

The number of diseases reported is not a true figure of the num- ber of the diseases that actually occurred in Douglas. Physicians and parents do not report the communicable diseases to the local Board of Health. State law requires that this must be done, but people are found very delinquent in reporting.
Tuberculosis:

Mantoux Tuberculin Skin Test:
On November 12 the Worcester County Health Association, in cooperation with the Douglas Board of Health and the School Com- mittee, sponsored the Mantoux Tuberculin Skin Test for first grade pupils. A total of 49 pupils were tested. One of these children was a positive reactor, which means that the child had been in contact with someone who had active tuberculosis. All members of the family were X-rayed, and recommendations made from the sani- torium will be followed up by the nurse.
Chest X-Ray Unit:
On March 22 the Worcester County Health Association, in co- operation with the Douglas Board of Health, sponsored a chest X-ray program at the Hayward-Schuster Woolen Mill. A total of 197 peo- ple were X-rayed on that day. These programs are made possible through the sale of Christmas seals each year.
BLOOD BANK
January 27, 1964, is the date scheduled for the Red Cross Mobile Unit to visit Douglas. I am presently in the process of organizing for this project
WELL CHILD CONFERENCE
On April 9 and 23, May 7 and 14, and on June 4, Well Child Conferences were held at the Town Hall from 10 to 12 o'clock in

the morning. Dr. Spooner examined 48 children at these confer- ences, which are conducted on an appointment basis. Parents are present with their children. Personal history is recorded on the school health record card for entering-school children. Vision-testing is done by local volunteers who have been certified by the State De- partment of Public Health. A dental hygienist from the State Health Department assists us by examining the teeth of the children and recommending the necessary referrals to a dentist.

PREMATURES
Three premature infants were born to mothers residing in Douglas. These cases did not qualify for financial assistance from the Douglas Board of Health.
SOCIAL SERVICE
The nurse, working in cooperation with the Welfare Department, the Police Department, state social workers, the teachers, and clergy, investigates many home conditions as to the need for emergency care, rehabilitation, etc. The nurse then communicates with various agencies whose functions are to assist us on a local level.

I attended local and state meetings pertaining to health, served as vice-president of the Worcester County Health Association, served as Representative Director of Massachusetts Tuberculosis and Health League, and served as Representative Director of the Worcester Chap- ter of the American Red Cross.
I was appointed as a delegate from the Worcester County Health Association to attend the International Union Against Tubeculosis which was held in Rome in September, 1963. This was a very unique educational experience. I had the opportunity of exchanging in- formation with delegates from 58 countries. I wish to express my appreciation to the Board of Selectmen for the material assistance and for the leave of absence granted to me in order to take part in this meeting in Rome.
Mr. Clifford Ballou has submitted a separate report pertaining to his functions as agent for the Board of Health.
I wish to especially thank the volunteer workers who have given hundreds of hours of their time in assisting at clinics, blood banks, and at all health projects sponsored by the Board of Health. They are to be commended for their community spirit. The time do- nated by the volunteers is a direct saving to the taxpayer.
The vaccine and other biologicals are kept under refrigeration without charge at Mr. John Frost's store and at the Goodness Store, and I wish to thank them for this privilege. I wish to thank the Board of Selectmen, the Salvation Army Service Fund Committee, the Red Cross Motor Corps ,Dr. Spooner, Mr. Clifford Ballou, the Red Cross Canteen Personnel, the Commercial Department at the high school, and everyone who assisted me in the health program.
