REPORT OF THE GUIDANCE DEPARTMENT
To the Superintendent of Schools:
I hereby submit my report for the school year 1962-1963.
A follow-up of the class of 1961 has been made and the return (better than 75%) has been most gratifying. Already, information is available from the students reporting back, which is meaningful and can be used in a manner which will be most helpful in our future curriculum plans and improved guidance services. This follow-up will be on a first, third and fifth year after graduation basis. That is, each graduating class will be polled three times. Such a plan works both to the advantage of the graduate and of course the school.
Mr. James McCarthy of the Junior High School is currently coun- seling all 7th and 8th grade pupils and will assist in the duties and responsibilities of the Guidance program.
We have been successful in setting up a pilot program in conjunc- tion with Northeastern University's Occupational Information Center. Two speakers have already talked to Juniors and Seniors. This pro- gram will continue through March. Northeastern University also pro- vides vocational tapes which can be most helpful within the classroom, making possible further depth to the Guidance program by means of classroom career discussions.
We are using as many measuring devices as possible to provide ability grouping. Therefore, use is made of appropriate tests, report

cards, the student's pattern of progress and, perhaps as significant as any one device, the considered opinion of the teachers.
Our testing program for grades 7 through 12 includes additional tests to those already used last year. They will serve as the basis for ability, achievement and interest. All grades (7 through 12) will be ad- ministered tests for certain specific purposes, (e.g. High School Place- ment Test for 8th graders entering high school; the use of Algebra and Latin Prognosis tests for pupils electing such subjects). The National Educational Development test, the National Merit Scholastic Qualify- ing test, and Preliminary Scholastic Aptitude test will be administered where appropriate and where the student specifically requests it. These tests are good predictors of success in terms of further education be- yond high school, but are used for guidance purposes, as well.
We are proud of the success many of the seniors have had in gain- ing admission to college. Colleges that admitted our seniors were: Parsons, Drew, Tufts, Children's Hospital, Bowdoin, Springfield, For- syth School of Dental Hygiene, Beloit, Buffalo, Northeastern, Univer- sity of New Hampshire, Rockford, University of Mass., Wheaton, Syracuse, Abilene Christian, Wentworth Institute.

REPORT OF THE SCHOOL HEALTH PROGRAM
To the Superintendent of Schools:
It is with pleasure I submit my first report as full time nurse in the Medfield Public Schools.
We are continually striving to prevent illness, to detect and correct physical defects that the students may have, and to instruct all in the teaching and practicing of normal emotional, physical, and mental well-being.

Dr. Stagg gives physical examinations to all students in the High School who are engaged in competitive sports and also to students who are due for their three year examinations. The parents are notified when and if a defect is encountered. After this notice has been sent home, the nurse later checks to make sure that medical attention has been given. It is our hope that eventually these examinations may be given by the family doctor and the results brought to school and made a part of the health record.
1. The Pure Tone Audiometer test is given to all students during the year. Retests are also given in the case of failure. If these tests are not passed, the parents are consulted, advised of the possible defect, and suggestions are made to them to have the student attended to by an ear doctor.
2. The Vision test is done by the Titmus Vision Tester. The Titmus testing apparatus is most compact and has self-contained units. Notices are sent to parents if a student fails to pass the test. This test is merely a screening process. If any student shows symptoms of eye difficulty, parents should not wait for the next school examination, but have medical attention when the problem is discovered.
The Mantoux Test (Tuberculin) has been given to most members of the Junior Class and Seniors who happened to miss the test last year.
The Dental Clinic is conducted at the Memorial School by Dr. Elliot Kirschbaum and the School Nurse under the supervision of the Board of Health. All elementary grade pupils through the fourth grade are examined and notices sent home when defects are found.
This is the second year of the "Future Nurses' Club" in the Med- field High School for all girls who are interested in becoming a nurse. This year, field trips were made to the Open House of Beth Israel Hos- pital, to the Children's Hospital and Framingham Union Hospital. These field trips give the girls an opportunity to talk with the student nurses and ask questions about nursing; they also had a chance to tour the hospitals and the nurses' homes.
Pre-school clinic is for the purpose of examining the children entering school for the first time and to discuss health problems with parents. It also gives the parent and child an opportunity to become acquainted with the school personnel and surroundings and to help the School Department ascertain and plan for the number of children entering school in September.

VITAL STATISTICS
IMPORTANT NOTICE
Chapter 46 of the General Laws of Massachusetts requires that every physician shall within fifteen days after the birth at which he has officiated, report the same to the Town Clerk of the Town in which the birth took place.
Parents within forty days after the birth of a child and every householder, within forty days after a birth in his house shall cause notice thereof to be given to the Town Clerk of the Town in which the birth took place.
Failure to give such notice is punishable with a fine.
Blanks for the return may be obtained upon application to the Town Clerk.
Parents and others are requested to carefully examine the follow- ing lists and if there are any errors or omissions, report such at once to the Town Clerk. This is important so that we may have as accurate and complete a record of Vital Statistics as possible.
